Committee Selection: Please mark your committee preference below. Note your choices as first, second and third. Your committee preferences are respected. However, as some areas fill, you may be placed on other committees as needed.

Schedule: Please note specific times you are available each day. It is appropriate to note "morning," "afternoon," "evening" or "all day." Shifts within these times will be scheduled with you at a later date.

Volunteer Uniform Package: All volunteers are required to purchase one Volunteer Package, which will provide the required uniform for the event. This year’s uniform will consist of a red golf shirt and cap/visor. Please complete the following information and include your payment with this application to help defray the tournament’s cost of providing these items.

Uniform Package Pickup Date: July 10, 2008.
